Description:
Provides information and referral to medical and dental providers for check-ups and follow-up care for Medi-Cal eligible persons age 20 or under. Persons age 18 or under from low-income families are also eligible for health examinations. Assistance with scheduling appointments and transportation is provided. Check-ups are available through private physicians, hospital outpatient clinics, community clinics and public health clinics. The multidisciplinary staff collaborates with a number of agencies to coordinate and enhance health services on behalf of children. Outreach and advocacy are provided in such areas as childhood lead poisoning prevention, immunization, asthma and the dental and nutritional needs of children.Location
