Wellness Checks / Medication Management

Service Description:

It’s not only our aging population—anyone who has health concerns that leave them unable to manage their medication regimen could benefit from a private nurse. Caregivers may be able to provide some in-home assistance, but are unable to manage medication and handle other medical tasks that are best suited to a nurse. A private nurse is able to help with the following: - Vital signs monitoring - Medication management - Wound assessment and care - Education to client, family, and caregiver - Frequent communication with care managers or family about patient status - Other services as requested Medication management services include: - Organize medications and assess ongoing medical needs - Develop medication regimen and checklist - Dispense correct dosage of each medication at the right time - Fill pill boxes or medication dispensers; update with new medication orders - Reconcile medications and notify client when refills are needed - Reorder medications as needed and procure refills - Check labels, expiration dates, and warn patient or caregiver about potential interactions

Post-Surgical Care (Wound Care)

Service Description:

Recovering from surgery, regardless of the type, can be physically and emotionally challenging for the patient and their family. It may not be possible for family members to take the necessary time off of work to care for a loved one after surgery. Caregivers can be a big help, but you may require an extra level of skilled care that can only be provided by a licensed nurse. Our nurses provide care to people of all ages—infants, children, adolescents, adults, and seniors—and support to families. We are committed to providing skilled care of the highest quality, with the goal of a successful post-surgical recovery for the patient and their family. Post-surgical services include: - Patient and family education – review discharge instructions to ensure successful recovery - Surgical site care, including wound care - Monitoring for signs and symptoms of infection - Pain management - Medication management, including administration of oral or IV medications according to doctor’s orders - Care team communication – contact patient’s doctor, surgeon, or other members of care team to revise care and provide status updates as necessary

Intravenous (IV) Therapies (Infusion Therapies)

Service Description:

NurseRegistry provides clinical and support services to individuals in need of infusion therapy. We match patients with skilled nurses experienced in a variety of different infusion therapies, resulting in shortened hospital stays, patient autonomy, and the elimination of visits to infusion clinics. Infusion therapy services include: - Immunoglobulin treatment for post-transplant or autoimmune patients - Antibiotic treatment for patients with severe infections - TPN – IV nutritional therapy for patients with GI disorders - Factor infusions for hemophilia - Port (or portacath) care and maintenance Teaching visits are an invaluable tool in helping a patient or caregiver feel more confident in their abilities to move forward independently with infusions.
