Provides (1) in-home private-duty nursing and attendant care,and (2) geriatric care management services, provided by geriatric specialist professional social workers.

Description:

A nonprofit, community-based provider of (1) In-home caregivers, providing personal care, private-duty care, companion care and attendant care and more (2) Geriatric care manager: geriatric specialist social workers providing comprehensive care needs assessments, care plan creation and implementation/oversight, medical services advocacy and more Caregivers are available on a 3- to 24-hour basis, seven days a week, short-term or long-term. Caregivers include nurse aides, most of whom have state certified training. Employees are bonded, screened and insured. Geriatric care managers are available on a scheduled basis as well as on an emergency basis when needed, subject to availabilty. Pathways Private Duty is part of a family of home care services affiliated with El Camino Hospital and Sequoia Hospital Dignity Health. The goal is to provide comprehensive, high quality services while making access to services easy for patients and families as well as physicians and others coordinating their care. Also known as Pathways Continuous Care.

Pathways Private Duty is a full service employer agency responsible for employee supervision, payroll taxes, worker’s compensation, liability insurances and unemployment taxes. Licensed, Insured and Bonded Caregiving Services Pathways Private Duty is licensed as a Home Care Organization under the California Home Care Services Consumer Protection Act. Pathways Private Duty caregivers are also all individually registered and background checked by the state as Home Care Aides.
