Description:Provides funding and case management for diagnosis and treatment for children age 21 or under with potentially chronic, disabling physical conditions. Also provides physical and occupational therapy for children age 21 or under with neuro-muscular, musculoskeletal or muscular diseases. There is no financial eligibility for diagnostic services and no financial eligibility for physical and occupational therapy.
