Description:
Home visiting program offering case management services to Medi-Cal clients who are either pregnant or have children up to age 5. Services include health screenings and linkages to resources/services, information on growth and development, referrals to recovery programs related to drug, alcohol and tobacco use, nutrition counseling, parenting classes and drop-in support groups. Public health nurses, community workers, social workers, nutrition staff and mental health specialists work together as a team. The toll-free line serves as a family resource line as well as an information line to nutrition and parent support.Location

Staff makes home visits. Client can request alternate location for visits. For parent support groups, varies according to meeting location.
