Core services include: primary medical care; individual, family and group counseling for concerns such as depression, suicidality, truancy, family issues, sexuality, substance abuse, eating disorders, and anger management; Outreach, individual and classroom health education, peer health education through Project PLAY (Peer Leadership Alternatives for Youth); Elements for Success (Elements): a school-to-career transition program focused on developing academic, leadership, and vocational skills for students ages 16-24 who have barriers to successful high school/GED completion and employment.

Service Description:

Our Primary Care Clinic Services: Check-ups and Primary Care Exams Immunizations/Vaccinations Tuberculosis skin test and treatment Vision and Hearing Screening Reproductive health exams, including Pap Smear and Breast Exams Contraceptives Emergency Contraceptives (“Plan B”) Pregnancy Tests Options counseling Sexually Transmitted Infections (STI) Tests including HIV Tests and Counseling Behavior health counseling services: Individual, family, couples, and group counseling. Counselors are here to help you to feel comfortable, safe and understood. In a counseling session you can talk about whatever you need to talk about, however you want to talk about it, and our counselors are here to support and listen to you. Health Education: reproductive health and pregnancy prevention education program, currently implemented in the JUHSD high schools. Our Health Educators and Peer Health Educators provide a ten day, comprehensive sex education workshop in all of the health classes within the Jefferson Union High School District.

Fees:

Medi-Cal, CHDP, FPACT, Healthy Families/Healthy Kids accepted if available. Otherwise no fee. Private insurance not accepted.

Youth and young adults age 13-24

Eligibility:

If age 17 or under, must have a parental consent form on file (except for confidential reproductive health visits)
