Description:
Offers an outpatient health center located on the first floor of the San Mateo Medical Center and designed for older adults in San Mateo County. The Center includes both primary care and geriatric assessment. The primary care clinic is for adults age 60 or over who either do not have a primary care provider or whose chronic or complex health issues would benefit from the time and resources clinic staff can offer. Physicians and nurse practitioners provide primary care. The multidisciplinary team, which includes nurses, a psychiatrist, psychologist, licensed clinical social worker and rehabilitation therapist, provide counseling and ongoing care management. The geriatric assessment program is a consultative service provided by a team of healthcare professionals. Patients are evaluated for physical health, mental health, social needs, and functional status. Older adults experiencing memory loss, confusion, recent falls, and frequent hospitalizations may benefit from this service. Staff will coordinate referrals to Aging and Adult Services and/or home and community based services.Location
