Provides social services to help stabilize clients who have just been released from jail.

Description:

Service Connect is an award winning multi-disciplinary service model that provides wrap-around services for the AB 109 and moderate to high-risk re-entry populations. Following a comprehensive needs assessment, all clients have access to a "safety-net bundle" comprised of short-term housing, clothing and transportation vouchers, phone cards and food. Under a collaborative model between Probation, HSA, Health and non-profit partners, clients are then connected to public assistance benefits, mental health and substance abuse services, peer-mentoring services, Moral Reconation Therapy programming, and family engagement support. Clients are also placed in rapid employment opportunities through the county's workcenter and receive employment services such as soft skills development and resume building, while gaining marketable job skills.
